Position Overview:

Under the direction of the Post Treatment Services Manager, the Post Treatment Case Manager is responsible for providing specialized mental health counselling services. This includes ensuring regular, appropriate documentation on all clinical files; regular conferencing/reviews of relevant cases; evaluating system effectiveness and recommending necessary changes; ensuring the current developmental, treatment, and psycho-social needs of clients are met by participating in the provision of assessment, counselling and referrals. A primary responsibility is to increase early identification of mental health problems and mental disorders. To ensure the provision of treatment plans and counselling services for clients that is consistent with the philosophy, policies, and standards established by Dilico Anishinabek Family Care.

Qualifications and Requirements:

A minimum requirement of an HBSW or related degree complimented by significant experience and demonstrated skill in related work will be considered. An MSW is preferred
Experience working with Anishinabek adults, families and communities
A minimum three (3) years adult mental health and addictions counselling experience with clients
Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications may be considered
Satisfactory Criminal Records Search; including a Vulnerable Sector Search
Ability to work flexible hours as determined by the Addictions Services Manager in relation to service requirements
Must posses a Class “G” Driver’s License and have access to a personal vehicle
A valid Standard First Aid/CPR Certificate is recommended

Hours of Work: 32.5 per week
